- Date of Death, Age at Death, Sex, Ethnic Group, and Marital Status
- Usual Residence: Country, State/City, Full Residential Address, and Occupation
- Place of Occurrence (Hospital, Home, etc.), State/Province, and Country of Death
- NPC Registration State, LGA, and Center
- Cause of Death, Doctor Certification Status, and Literacy Status
Required documents
- Identification of Deceased (NIN card, International Passport, or Voter's card)
- Identification of Applicant / Next of Kin (Valid ID card or NIN slip)
- Death Notification from Hospital (if death occurred in a hospital)
- Burial Permit (if deceased has already been buried)
