- Proprietor Full Name (Surname, First Name, Middle Name), Phone Number, Gender
- Proprietor NIN (National Identification Number) & NIN Date of Birth
- Proprietor Residential Address (State, LGA, City/Town, Full Street Address)
- If Partnership: Full Name, Phone Number, Gender, NIN, NIN Date of Birth, and Residential Address for each partner
Required documents
- Clear passport photograph (white background)
- Clear NIN slip
- Clear picture of signature signed on a plain white paper without lines
- If Partnership: Clear passport photograph, NIN slip, and signature for each partner
